Facial fat is usually pictured as a single layer sitting under the skin. It is not. It is organised into separate compartments, and they do not all respond to the same change in the same way.

So as overall body fat comes down, the face comes down with it, though not evenly. One area can shift noticeably while another stays much as it was. How the skin above those areas behaves varies too, influenced by age, by genetics and by how fast the change occurred.

None of this depends on the method. The pattern holds whatever route the weight loss took.

It is worth saying plainly that this is physiology rather than damage. Whether there is anything worth discussing, and whether this is even the right moment to discuss it, is something we work out individually. Sometimes the answer is to wait.

Layer four of five: the muscles.

They are what let a face move, and every expression a person makes comes from them working. Movement is normal, healthy function.

Anything to do with facial movement is a clinical topic and belongs in a consultation with a qualified registered health practitioner, so this one stays with the anatomy and nothing else.

Layer three of five: the facial fat.

It is not one soft sheet under the skin. It sits in separate compartments at different depths, and they are what give a face its softness and its contours. How full they are and where they sit both change over a lifetime, which is normal biology.

It is where collagen and elastin are found, the proteins behind the support and stretch in skin, and it holds a good part of the water skin carries. Like every tissue in the body, it changes over the course of life.
