Trying to make one understand their skin/hair needs better!

Why does your melasma keep coming back?

You may be treating the pigment—but missing what keeps reactivating it.

Hormones, pregnancy, visible light, irritation, the wrong diagnosis and even unregulated “lightening” creams can all play a role.

In this carousel: what melasma really is, why brown patches are not always melasma, why tinted sunscreen with iron oxides may matter, and which treatment shortcuts can backfire.

Save this for your next dermatology appointment. Share it with someone treating every dark patch the same way. Follow for evidence-based dermatology for skin of colour.

Warum kommt dein Melasma immer wieder?

Unfortunately, I do love prescribing evidence-based dermatology.

Not every skin concern needs another viral product, a complicated 12-step routine, or an expensive treatment package.

Sometimes, the right treatment is a well-studied medical option selected for the right patient, after the right diagnosis:

• Oral tranexamic acid may be considered for selected patients with melasma.
• Spironolactone can be useful for hormonal-pattern acne in appropriate women.
• Rosacea may benefit from targeted prescription combinations such as metronidazole, ivermectin, or azelaic acid.
• Selected patients with menopausal pattern hair loss may benefit from a carefully monitored combination approach.
• Persistent perioral dermatitis often needs diagnosis-specific treatment rather than more skincare products.
• Laser hair reduction may be considered as part of a treatment plan for selected patients with hidradenitis suppurativa.

The key is not simply knowing the name of a medication or procedure. The key is knowing who needs it, who should avoid it, what needs to be monitored, and what the alternatives are.

That is why I love modern dermatology: when used thoughtfully, it can do more than improve the skin. It can help patients feel comfortable, confident, and like themselves again.

Which treatment surprised you most? Comment below and save this for your next dermatology appointment.

Follow for evidence-informed dermatology, skin of colour, hair-loss education, aesthetics, and honest treatment discussions without the hype.
