Dietary Management of Type 2 Diabetes

*Portion Size*
This is very essential if you must have a hold on your blood sugar level.
Portion size is everything pertaining to your meal. There are several ways to go about portion sizes. It ranges from carbohydrates counting to simple tools that can be used to estimate what your portion size should be.
Note: if you are on insulin, it is essential to go for carbohydrates counting and match your meal with your dosage.
But if you are not on insulin or medication that increases insulin secretion then the following may be very helpful but if you want to be very accurate and ensure that what you eat matches up with your lifestyle maybe very active, active or sedentary, consult your dietitian.
But the following methods are also very helpful, trust me๐Ÿ˜ .
*For your carbohydrates*
The size of your carb should not be bigger than your own fist.

*For protein*
This should be the size and thickness of your palm. Somebody may say, woow its very big. Well, remember I told you that insulin is a protein so eating enough protein helps in the uptake of blood sugar.
Now this may not necessarily be your whole meat/ fish, it just means that while you may not be able to afford to eat that size of protein per meal. You can include other source of protein like crayfish, soya milk, locust beans, liver etc alongside your fish or meat. And if want to eat meat please๐Ÿ™๐Ÿพ let it be a lean meat (without fat) or birds like chicken without skin remember I told you fat may interfere with glucose uptake.

*For fat*
For each of your meal, your fat/ oil should not be more than the size of your thumb. Someone says what if I want to prepare soup that I will eat for about 3 days. Its simple, estimate the number of times you are likely to eat it per day multiply by the number of days the soup may last.

This is a model that is recommended to assist with implementing and sustaining skills and behaviours needed for ongoing self management.
This is to ensure that diabetic patients have the right knowledge to manage their diabetes and also have the adequate support from medical team, family and caregivers.

I am sure we have some caregivers here, see to help your relative improve his/her blood sugar you must support them adequately.

Don't cook their food separately, choose positive, strength based words and phrases, avoid judgmental words because the impact language has on building therapeutic relationship is enormous. Don't say " you cannot eat what we are eating because you are diabetic," I tell you it has a psychological effect on them.

All foods contain carbohydrates have sugar so don't make life boring for them. Let them eat freely, the only thing is they must know the quantity adequate for them, so ask your dietitian๐Ÿ˜˜

Hypoglycemia
This is when your blood glucose levels is lower than 70mg/dl, however, this may be a bit higher for children and the elderly.
If you are on metformin, you are not likely to have hypoglycemia. While we do not want you to have hyperglycemia, we do not want to go too low that you end up being "hypo". Hypoglycemia can as well result in serious complications brain damage for children, weakness, seizure and death.
Similarly
1. Mismatching of insulin/drug with food may cause hypoglycemia: if you have high dosage compared to your meal, it will drain everything off resulting in hypoglycemia.
Also, if you are on insulin or insulin secretagogues and you have a strenuous exercise, you could go hypo.
So, make sure your food is matched with your lifestyle and medication to avoid hypoglycemia.

Let's talk about hyperglycemia in Diabetes

Sometimes you check you blood sugar level and you find it is high.

Note: blood sugar is high when the fasting blood glucose level is above130mg/dl usually before breakfast or it is above 180mg/dl 1-2hours after a meal.

However, your endocrinologist or doctor should determine what your normal blood glucose level should be because factors like age, pregnancy etc may influence what your normal blood glucose level should be.

Hyperglycemia can lead to serious complications like heart problems, nerve problem, kidney damage, or eye problem. So, we want to prevent hyperglycemia by all means.

Hyperglycemia could be caused by:
1. Mismatching of insulin/ drug with your food: let's say you are on a rapid acting insulin like lispro or short acting insulin.
The rapid insulin peaks at about an hour while short acting peaks between 2-4hours.
Now if you take a high fat meal let's say akara with this kind of insulin, by the time the insulin peaks, the food is still in the stomach because it takes longer time (not less than 4hours) to digest high fat meals so the food is most likely getting into the blood stream when the insulin has relapsed leading to hyperglycemia.

2. Skipped/delayed meal: if you skip your meal or you wait till later to eat, your body may not be able to handle the glucose in the food because you may more likely eat more leading to hyperglycemia.

3. Irregular eating pattern: lastly that I am going to talk about, if you are fond of eating anyhow, let's say today you ate your breakfast early, tomorrow you decided to take a brunch etc. Your body will not metabolize the food properly. It takes about 4hours for a proper meal to move from the stomach to the bloodstream. So you may want to space your meal time to every 4-6hrs. This again differ from one person to another. It is cool you speak with your dietitian and have the appropriate time suitable for you planned together.

Things to unlearn about diabetes management cont'd

*Hunger*: diabetic patients are very good at managing hunger, they feel hungry they don't want to eat, they just want to endure till later in a bid to control their blood sugar๐Ÿคฆ๐Ÿฝโ€โ™€๏ธ๐Ÿคฆ๐Ÿฝโ€โ™€๏ธ. Listen, there is a lot going on in your body. Your line of management influences how you use food. If you are on insulin, don't dare because it will peak and when it does and there is no food there or you are on medication that helps your body produce more insulin like daonil mehn...there is problem you will run low on sugar (hypoglycemia). Trust me this is not good at all, we will get to talk about it later.
Note: don't starve yourself. There is always allowance for snacks.
