‎
‎Seconds Away From a Fatality Today
‎
‎Toolbox talk was done. Permit signed. JSA reviewed. We were rigging a 3-ton valve spool on deck level. Clear weather, no rush.
‎
‎Peter was signalman. Rigger John checked the slings — or so we thought. One 2-leg wire rope sling, looks fine from 5 feet away.
‎
‎Load goes up 6 feet. Then I heard it.
‎
‎PING.... not loud, but you know that sound. One leg of the sling parted like thread. 3 tons dropped straight down.
‎
‎It missed the deck crew by 2 meters. Two. Meters.
‎
‎The spool hit the deck and bounced. If Bode had taken one step left to pick up his tagline… we’d be doing a fatality investigation, not a near miss report.
‎
‎What we found after:
‎The sling had broken strands inside, hidden under dirt/grease. Last inspection was “visual” 3 months ago. No load test record.
‎
‎Lesson for all of us:
‎1. “Looks fine” kills. Corrosion hides. Do thorough pre-use checks — run your hand _with gloves_ along the whole sling.
‎2. Paperwork ≠ Safety. JSA doesn’t stop gravity. Stop and check if your gut says something’s off.
‎3. Near miss = gift. Next time it won’t miss.
‎
‎We all went home today. But only by 2 meters.
‎

STOP AND THINK BEFORE YOU ACT!

One unsafe decision can change a life forever.
Before starting any job, ask yourself: 👉 What can go wrong?
👉 Do I have the right PPE?
👉 Is the equipment safe?
👉 Do I understand the job and the hazards?
👉 What will I do if something goes wrong?
Safety is not about stopping work. Safety is about making sure everyone goes home safely. 💪🏽
Never be afraid to STOP THE JOB when you see an unsafe act or condition. Your voice could save a life.
💬 Question: If you see a colleague doing something unsafe, would you speak up—even if they are your supervisor?
Speak up. Stay alert. Go home safe. 🦺
