💊 Pharmacology Challenge #2
A client with atrial fibrillation is receiving warfarin. The client reports starting a new medication prescribed for a urinary tract infection. The nurse reviews the client’s laboratory results:
* INR: 5.2
* Hemoglobin: 11.8 g/dL
* Platelets: 210,000/mm³
The client has no obvious signs of bleeding.
Which action should the nurse take FIRST?
A. Administer the prescribed warfarin dose and recheck the INR tomorrow.
B. Hold the warfarin and notify the healthcare provider.
C. Administer vitamin K immediately.
D. Encourage the client to increase foods high in vitamin K.
👀 Think carefully—the absence of bleeding does NOT mean the INR is safe.
Drop your answer: A, B, C, or D. 😎🫡

🧪 MUST-KNOW NCLEX LABS 🧫
Number 6: BUN & CREATININE 🫘
🧠 What do you need to know?
Both can give you information about kidney function, but creatinine is more specific for renal function.
🧪 BUN: commonly about 10–20 mg/dL
🧪 Creatinine: commonly about 0.6–1.3 mg/dL
💡 Don’t get stuck memorizing the exact range—the lab’s reference range may vary.
🚨 NCLEX TIP
When creatinine rises, think:
“The kidneys may not be filtering properly.”
🔥 LET’S SEE IF YOU CATCH THE TRAP
The nurse reviews the following laboratory results:
* Client 1: BUN 32 mg/dL, creatinine 1.0 mg/dL
* Client 2: BUN 18 mg/dL, creatinine 2.1 mg/dL
* Client 3: BUN 24 mg/dL, creatinine 1.1 mg/dL
* Client 4: BUN 14 mg/dL, creatinine 0.9 mg/dL
Which client should the nurse be MOST concerned about?
A. Client 1
B. Client 2
C. Client 3
D. Client 4
👉 BUT HERE’S THE TRAP:
Don’t automatically choose the client with the highest BUN.
Look at both values and ask yourself:
“Which result tells me the kidneys may not be doing their job?”
👇 What’s your answer?
I hope you’re saving and sharing these post with your study partners! This might just be your exam question 🫡

💊 Pharmacology Challenge 💪
🤔How well do you know your pharmacology? Expect to see a couple of them in your exam. So get ready!
👉 A client with heart failure is prescribed digoxin and furosemide. The nurse reviews the morning laboratory results before administering the medications.
Which finding requires the MOST immediate action?
A. Heart rate 58/min
B. Potassium 2.8 mEq/L
C. Sodium 134 mEq/L
D. Blood pressure 104/66 mm Hg
What’s your answer? Drop it in the comments and tell us why.

🎯 NCLEX QUESTION STRATEGIES 🫡
The NCLEX is not an exam designed to fail you! You just have to understand what is expected of you.
Before you sit for the NCLEX, you MUST know how to interpret the question.
Sometimes, the problem isn't that you don't know the nursing content.
It's that you misunderstood what the question was asking.
✅ Know the difference:
🔹 FIRST: What action should happen before the others?
🔹 PRIORITY: Which patient or problem requires the greatest urgency?
🔹 BEST: Which option is most appropriate in the given situation?
🔹 IMMEDIATE INTERVENTION: Which finding requires prompt action to prevent harm?
🔹 SATA: Select every option supported by the question. Do not stop after choosing one or two.
🧠 NCLEX TIP:
Before looking at the options:
Identify the actual problem.
Determine whether the patient is stable or unstable.
Identify what the question is asking.
Eliminate unsafe or irrelevant options.
Choose the answer supported by nursing principles.
🎯NCLEX CHALLENGE
A nurse is caring for a patient receiving IV insulin for diabetic ketoacidosis (DKA). The patient's blood glucose is now 58 mg/dL.
What is the PRIORITY action?
A. Continue the insulin infusion and recheck glucose in 30 minutes.
B. Stop the insulin infusion and initiate treatment for hypoglycemia according to protocol.
C. Administer the scheduled subcutaneous insulin.
D. Document the result as an expected finding during DKA treatment.
Don’t rush to the options immediately. Think about the question, understand it and then look at the options.
Drop your answer! 👇and tell us why you chose that option ✅

🧪 MUST-KNOW NCLEX LABS 🧫
Number 4: MAGNESIUM (Mg²⁺)
✅ Normal magnesium: approximately 1.7–2.2 mg/dL
🔻 LOW magnesium — Hypomagnesemia
Think:
* Tremors
* Muscle cramps
* Hyperactive reflexes
* Seizures
* Dysrhythmias
🔺 HIGH magnesium — Hypermagnesemia
Think:
* Decreased deep tendon reflexes
* Muscle weakness
* Hypotension
* Respiratory depression
* Cardiac problems 🚨
💡 NCLEX TIP:
HIGH magnesium = everything slows down. 🐢
Think ↓ reflexes + ↓ respiratory rate + ↓ blood pressure.
👉Now attempt this question and give the reason for your answer👇
The nurse is caring for a client receiving IV magnesium sulfate. Which finding requires the nurse to act FIRST?
A. Magnesium 3.1 mg/dL, deep tendon reflexes +1, respiratory rate 16/min
B. Magnesium 4.2 mg/dL, deep tendon reflexes absent, respiratory rate 10/min
C. Magnesium 2.8 mg/dL, blood pressure 104/68, respiratory rate 18/min
D. Magnesium 1.5 mg/dL, deep tendon reflexes +3, respiratory rate 20/min
Remember, don’t make the abnormal lab the answer. Make the clinical reasoning the answer ✅
Looks like all my NCLEX loves💖 here are lab gurus 🤔 cos I don’t understand the engagement on these lab topics 😀
Should we change the topic my people 🤔😀

🧪 MUST-KNOW NCLEX LABS 🧫
Number 3: CALCIUM (Ca²⁺)
👉 Total calcium: approximately 8.5–10.5 mg/dL
🔻 LOW calcium — Hypocalcemia
Think:
* Tingling/numbness
* Muscle cramps
* Tetany
* Chvostek’s sign
* Trousseau’s sign
* Seizures 🚨
🔺 HIGH calcium — Hypercalcemia
Think:
* Weakness
* Constipation
* Kidney stones
* Confusion
💡 NCLEX TIP:
Low calcium = nerves and muscles become more excitable⚡
So when you see tingling + muscle spasms/tetany, think hypocalcemia.
✅ NCLEX QUESTION:
A nurse is caring for four postoperative clients. Which finding requires the nurse’s priority intervention?
A. A client with a calcium level of 7.8 mg/dL reports numbness around the mouth and has a positive Trousseau sign.
B. A client with a calcium level of 11.1 mg/dL reports constipation and decreased appetite.
C. A client with a calcium level of 8.1 mg/dL reports tingling in the fingers after thyroid surgery.
D. A client with a calcium level of 10.9 mg/dL reports increased thirst and urination.
Attempt the question guys and give us your reason in the comments 👇
Our goal is to pass the Nclex in one sitting ❤️

✅ This question has been causing a lot of debate so I decided to bring it to my Nclex Gurus here 😀
🚨 NCLEX DEBATE TIME! 🚨
This one looks simple… until you start thinking about how NG-tube placement is actually verified.
A newly inserted NG tube is ready for feeding.
What should the nurse do FIRST?
👇 Drop your answer: A, B, C, or D
And don’t just drop the letter—tell me WHY. 😎
Let’s see what the NCLEX minds in this community are thinking! 🧠🔥
No peeking at the rationale yet! 😂
