As a Nurse Practitioner β here is what I check when a woman cannot lose weight no matter what she does. π
Because when a woman comes to me frustrated, doing everything right, and still not seeing the scale move β the answer is almost never that she needs to eat less or work out more.
It is almost always in her labs.
Specifically the labs nobody ordered. π§¬
First β I check her fasting insulin.
Not just her blood sugar. Insulin. Because fasting insulin is the earliest sign of insulin resistance β and insulin resistance means her body is in constant fat storage mode regardless of how clean she eats or how hard she trains. This single marker explains more stubborn weight gain than almost anything else I test.
Second β I check her Free T3.
Not just TSH. Free T3 is the active thyroid hormone that actually drives her metabolism at the cellular level. You can have a perfectly normal TSH and a Free T3 sitting at the very bottom of the range β meaning her metabolism is running at a fraction of what it should be. Most providers never check it. π€
Third β I check her cortisol pattern.
Not a single morning draw. A full pattern. Because cortisol that is chronically elevated β from stress, under eating, over exercising, or poor sleep β tells her body to store fat in her midsection around the clock regardless of what she eats.
Fourth β I look at her gut.
Because a disrupted gut microbiome directly impairs how her body metabolizes estrogen, manages inflammation, and regulates insulin sensitivity. You cannot out-diet a gut that is working against your metabolism. π§¬
Fifth β I check her estrogen metabolite ratio.
Not just her estrogen level. How her body is processing and eliminating estrogen. Because estrogen dominance drives fat storage β specifically in the belly, hips, and thighs β and it shows up even when estrogen looks normal on a standard panel.
None of these show up when your doctor says your labs are normal.
All of them show up when someone actually looks. π€
This is root cause medicine.
And this is why the women I work with finally start losing weight after years of being told nothing is wrong.

Because if you have been eating less, working out more, and your belly fat is still not moving β your labs are telling a story that nobody has read to you yet.
Here is what I almost always find when I run functional testing on a woman with stubborn belly fat that will not respond to diet or exercise:
Fasting insulin that is technically normal but high enough to keep her body in constant fat storage mode β driving cravings, energy crashes, and a metabolism that fights every effort she makes. π§¬
A Free T3 sitting at the very bottom of the normal range β meaning her thyroid is technically fine on paper but her metabolism is running at a fraction of what it should be.
Cortisol that spikes at night instead of dropping β telling her body to store fat in her midsection as a survival response while she sleeps. Every single night.
And a gut microbiome so disrupted that her body cannot properly regulate estrogen, manage inflammation, or absorb the nutrients it needs to actually burn fat efficiently. π€
None of these show up when your doctor says everything looks normal.
All of them show up when we actually look deeper.
Cutting calories did not fix any of this for my clients.
Finding the root cause did. π§¬

The next time your doctor tells you your labs are normal β ask them what your fasting insulin, Free T3, and estrogen metabolite ratio actually are and watch how fast the conversation changesβ¦ or find a new doctor. π
I say that with full respect for the medical profession I am a part of.
But I also say it as a Nurse Practitioner who has sat across from hundreds of women who were handed a piece of paper that said βwithin normal limitsβ while their body was quietly falling apart. π€
Here is what most conventional providers are not checking β and what those missing labs actually reveal:
Fasting insulin β not just your blood sugar. Insulin is the earliest sign of metabolic dysfunction and it can be driving your belly fat, your cravings, and your energy crashes for years before glucose ever becomes abnormal on a standard panel.
Free T3 β not just TSH. TSH tells us your thyroid is being asked to work harder. Free T3 tells us whether your cells are actually receiving the active thyroid hormone they need to drive your metabolism, your energy, and your body temperature. You can have a perfect TSH and a completely dysfunctional thyroid. π§¬
Estrogen metabolite ratio β not just your estrogen level. How your body breaks down and eliminates estrogen determines whether it becomes inflammatory, whether it drives hormonal acne and heavy periods, and whether it increases your long term cancer risk. This is never checked in conventional medicine. Ever.
Normal is not the same as optimal.
And optimal is what you actually deserve. π€

I could not fall asleep until 1am and was wide awake by 5am every single night for years. π
Melatonin did absolutely nothing.
And for the longest time I just accepted that I was a bad sleeper.
It was not until I actually tested my cortisol and my hormones that I found out what was really going on.
My cortisol was spiking at night instead of dropping.
My progesterone was too low to calm my nervous system down.
And my magnesium was so depleted my body had no way to wind down no matter what I tried.
Nobody had ever tested any of that. They just handed me melatonin and sent me home. π
Once I addressed all three I was asleep by 10pm within two weeks.
As a Nurse Practitioner I now know that most women struggling with sleep are not bad sleepers.
They are depleted. They are hormonally imbalanced. And they have been given the wrong solution for the wrong problem. π§¬
