Possible Sensations at Trigger/Needle Points After Dry Needling
After needle removal, the treated points and surrounding tissues may continue to experience various sensations. These are generally categorized into normal physiological responses and abnormal reactions that require attention:
1. Normal Residual Sensations and Recovery Phase (Usually Subsides Within 24–48 Hours)
* Residual Soreness and Aching (“Post-Treatment Soreness”):
* After needle removal, a faint aching, soreness, or mild dull pain may persist locally. This occurs because the needle stimulates deeper myofascial layers and muscle fibers, while neural and tissue regulation continues.
* Management: No special treatment is required. Adequate rest and keeping the area warm are recommended.
* Mild Subcutaneous Bruising / Discoloration:
* Needle points are rich in tiny blood vessels, and occasional minor bleeding after needle withdrawal can form a small bruise.
* Management: Apply a cold compress within the first 24 hours (to minimize bleeding). After 24 hours, mild warm compresses can be used; the bruise will typically resolve on its own within a few days.
* Systemic Relaxation and Fatigue:
* Feeling deeply relaxed, sleepy, or experiencing mild general fatigue after the session. This is a sign of parasympathetic nervous system activation as the body enters a state of self-repair and autonomic regulation.

"De Qi" is a key indicator of effective treatment, historically referred to as "the arrival of Qi brings therapeutic effect." During De Qi, both the patient and the practitioner experience a mutual dynamic sensation:
1. The Patient's Subjective Sensations
* Soreness (Aching/Fullness): The most common sensation, usually occurring in muscular areas (such as ST36/Zusanli or LI4/Hegu). Patients feel a deep, aching heaviness within the muscle layer.
* Numbness (Tingling Sensation): Similar to a faint electric current passing through or localized numbness, often radiating along neural or myofascial pathways (referred sensation).
* Heaviness: The needle insertion site or the entire limb feels exceptionally heavy, as if weighted down.
* Distension (Fullness): A sensation of outward expansion or increased tension surrounding the point.
* Warmth/Coolness: In sensitive individuals or with specific manipulation techniques, localized warmth or a cool breeze sensation may spread at the site or along the pathway.
2. The Practitioner's Tactile Sensation
* A distinct feeling of tightness, resistance, or a "grasping" effect around the needle, traditionally described as "a fish biting the hook." This indicates that the local tissue response and neuromuscular reflexes have been successfully engaged.
