A facelift should not look like a facelift.
For many patients, that is the real concern. Not whether surgery can make them look younger, but whether they will look operated on afterward.
That concern has history behind it.
Earlier facelift techniques often relied heavily on pulling and tightening the skin. The problem is that skin is not what holds the face in its natural position. When too much of the result depends on tension at the surface, the face can begin to look stretched, flattened or simply different from itself.
Modern facial surgery looks deeper.
With a deep plane approach, the goal is to work with the underlying anatomy, including the deeper tissues and retaining ligaments, and reposition them more closely to the direction from which they descended. The skin can then be redraped rather than relied upon to carry the lift.
The distinction is important.
The goal is not to make a face tighter. It is to restore its structure while preserving the features that make that face recognizable.
When considering a facelift, one of the most useful questions you can ask your surgeon is:
What is actually holding my result?
The answer can tell you a great deal about the operation being proposed.

Revision Facelift
If you had a facelift some years ago and have begun to consider a second procedure, this is worth reading.
One of the most common concerns following an earlier facelift is the persistence of a heavy neck. Older techniques did not always address the deeper structures beneath the platysma muscle, including the submandibular glands. For many patients, these structures need to be addressed to achieve a more defined and lasting improvement.
Dr. Anzarut has performed deep neck surgery for more than a decade and was among the early pioneers of this approach in Canada.
Another common concern is persistent hollowing beneath the lower eyelids. In some patients, restoring volume can help replace the facial fat that has diminished with age. Dr. Anzarut pioneered the use of custom tear trough implants in Canada to provide significant and carefully tailored improvement in this area.
Revision surgery is different from a first facelift. The surgeon is working with scar tissue, previous incisions, and tissue that has already been operated on and healed. This makes the procedure more complex and means it must be evaluated and planned as its own surgery.
Dr. Anzarut begins by assessing when the original surgery was performed, the location of the existing scars, the condition of the tissues today, and what is specifically concerning the patient. Only then can a thoughtful and individualized plan be developed.
There is no urgency in any of this. The first appointment is a conversation, not a decision.

Dr. Anzarut has been a plastic surgeon on this part of the Island since 2011, when he founded the service at Cowichan District Hospital and became the sole provider for a community of close to one hundred thousand people.
What is less well known locally is where he trained before he arrived.
Four countries. Canada, the United States, Belgium and Australia. More than five years of fellowship training after completing a full plastic surgery residency, and board certification in both Canada and the United States.

When evaluating a facelift or neck lift, one of the areas to pay close attention to is the submental region, the area underneath the chin.
A well-executed result should create a clean, natural transition from the jawline into the neck. Yet when looking at before-and-after photos online, this area is often difficult to assess, or not shown at all.
If you’re considering a facelift or neck lift, don’t just look at the before-and-after. Look at how the entire face, jawline and neck work together.
You will never get long-term structural results from facial massage or facial exercises. Never. Depuffing? Yes. Improved circulation? Sure. But lifting tissue, restoring volume, changing the foundation of your face? Not possible. Calling it a facelift alternative isn’t a wellness tip, it’s misinformation.
A facelift restores shape and structure but the skin tells the whole story.
