Today I experienced my first positive aspiration during a filler treatment.
For anyone who isn’t familiar with the term, a positive aspiration occurs when you pull back on the syringe plunger before injecting and blood enters the syringe. This can indicate that the needle or cannula is within a blood vessel, which means injecting filler in that position could potentially cause a vascular occlusion.
As soon as I got the positive aspiration, I remained calm, stopped, changed the syringe of filler, and reassessed my position. I then continued treatment by moving slightly away from the area where I had previously aspirated.
This experience reinforced just how important proper training is. Had I not been thoroughly trained, I may not have known how to recognise a positive aspiration or how to respond appropriately. Although this was my first positive aspiration, and hopefully my last, it was reassuring to know that my training prepared me to manage the situation safely and confidently.
Aesthetic treatments are about far more than achieving beautiful results. They require a detailed understanding of anatomy, risk management, and complication prevention. Choosing quality training isn’t just about learning how to inject — it’s about knowing exactly what to do when something unexpected happens.
Today’s experience was a valuable reminder that education, preparation, and staying calm are some of the most important tools we have as practitioners.
