She went as a model on a training day for “Russian lips”.

When she came to us we assumed a student had done it. It was the trainer.

She was told it would settle.

It was never going to settle. This wasn’t swelling — it was product placed where her lips were never going to carry it.

Then it got worse. The same practitioner offered to dissolve it. They weren’t a prescriber, and she was never offered a consultation with one. Hyaluronidase is a prescription-only medicine. If nobody in that room could prescribe it, where was it coming from? We’ll let you sit with that.

“Russian lips” is a technique, not a look you order off a menu. In the right anatomy it can be beautiful. Hers wasn’t — smooth, tight tissue with very little natural space. When a lip won’t take volume, the answer is never more volume.

We assessed her, dissolved using proper protocols and testing, then waited two weeks. When she came back the job was deep structural placement and shape, kept conservative. Doing lips well means reading the anatomy in front of you and being honest about what it can hold. Sometimes that means saying no.

We see this constantly. People chasing a deal, signing up as models for treatments they’ve seen online. There’s nothing wrong with being a model — training needs models. But it doesn’t mean you accept a lower standard. You’re still a patient.

Ask before anyone treats you: Are you GMC, NMC or GDC registered? Which product, and can I see the batch? Who prescribes here, and have I met them? What’s your emergency protocol? If this needs reversing, who prescribes it?

If those questions make the room uncomfortable, walk out.

This is what puts people off lip filler. Not filler — bad filler, by people calling themselves trainers. We’re angry about this one.

We didn’t make this nose smaller. Not by a millimetre.

That’s the part nobody expects.

Look at the profile on the left — your eye catches on the dorsal hump, and the tip sits slightly downturned.

On the right, the line runs straight and the tip sits a touch higher.

Nothing was removed to get there.

Small, precise volumes of hyaluronic acid were placed above and below the hump, so the profile reads as one smooth line instead of a bump. A tiny amount of support at the base lifts and rotates the tip.

The nose is technically fractionally bigger. It just doesn’t look it, because light now travels across it differently.

Camouflage, not reduction.

That’s the whole principle — and it’s exactly why this treatment suits some noses beautifully and is completely wrong for others.

An honest consultation matters here more than almost anywhere else on the face.

Worth saying plainly: the nose is one of the highest-risk areas in facial aesthetics. The blood supply is unforgiving. Anatomy, product choice, injection depth, and knowing when to stop — or when to decline altogether — are the entire job. This is not a treatment to shop around on price for.

Let’s talk about the chin ❤️

It’s one of the most overlooked features in facial balancing — but it plays a huge role in how the whole face reads.

Here’s why. The chin anchors the lower third of the face. When chin projection is slightly set back, it can make the nose appear more prominent, shorten the appearance of the jawline, and reduce definition where the face meets the neck. So even though people often focus on the nose or jaw, the chin is frequently the missing piece.

For this client, we placed a small amount of dermal filler to gently enhance projection and improve balance across the lower face. Notice how the front-on proportions look more harmonious — not because any single feature has been dramatically changed, but because the relationship between features is now better aligned.

This is the principle behind facial balancing: we assess your face as a whole and treat the area that will make the biggest difference, rather than chasing one feature in isolation. Sometimes that’s lips or cheeks. Often, it’s the chin.

A few things worth knowing:

The tear trough — that hollow, shadowed area under the eye that leaves you looking permanently tired — is one of the most technique-sensitive areas on the entire face. The anatomy is delicate, the margin for error is small, and it’s not an area to hand to someone just because their reel looked good.

In the tear trough world, before and afters are notoriously manipulated. The oldest trick in the book: have the client tilt their head up for the “before” so the hollow throws a deep shadow and looks its worst, then straight-ahead or slightly down for the “after” so it all but disappears — a “result” you’d get with no treatment at all, just a change of angle. Add a harsh ring light, a filter or two, and you can sell a transformation that never actually happened.

Let’s clear this up, again: filler isn’t the problem 😖

Almost every day, someone sits in our chair and tells us they ‘don’t want filler’ because of something they saw online, or a “botched” result they’ve heard about.

Everyone thinks they know what killed him. They’ve got it wrong.

A senior beauty-industry executive died this year of a pulmonary embolism, and his death was linked to cosmetic filler. Social media did the rest: facial filler can stop your heart. This is why I’ll never get my lips done.

So I did what the headlines didn’t. I read the medical examiner’s actual ruling — then read every documented case of filler reaching the lungs in the published global literature.

Here’s what I found, and what I haven’t seen one other clinic say out loud:
▪️ The ruling confirmed filler caused the embolism. It did NOT say where on his body the filler was. That detail is missing from every report.
▪️ In all of published medicine, there is not one recorded death from a pulmonary embolism caused by FACIAL filler. Not a single case.
▪️ The deaths that do exist are large-volume BODY procedures — buttock, breast, ge***al filler. Often permanent materials. Very often performed by someone with no medical training at all.

The distinction the headlines erased is the entire story. It’s the difference between being frightened of the wrong thing and understanding the real one.
