My gorgeous client came to me a few years ago after three surgical rhinoplasty procedures, and despite everything she had been through, she still wasn’t happy with the appearance of her nose.

One thing many people don’t realise is that previous trauma or surgery can completely change the anatomy of the nose. Scar tissue, altered blood supply, and changes to the underlying structure can make a non surgical rhinoplasty significantly more complex. Many practitioners will understandably decline these cases because they carry a higher level of risk.

My client had been coming to me for other treatments for some time, and we built a trusting relationship. After a full assessment and discussion of the risks, we decided to take a very cautious approach.

The aim wasn’t to create a “perfect nose.” It was to restore balance. Her previous surgery had removed too much bone across the bridge, leaving the nose looking sunken and flat, which made the tip appear out of proportion.

Over the last few years, we have used tiny amounts of filler, placed gradually and strategically across the bridge. We carried out the initial treatment, reviewed after two weeks, and then left months between sessions before adding any more. Slow, conservative treatment is often the safest approach in complex cases.

I also want to use this post to raise awareness.

A non-surgical rhinoplasty is often marketed as a quick lunchtime treatment, but it is one of the highest risk filler procedures we perform. The nose has a complex vascular network, and complications can be serious.

When a non surgical rhinoplasty goes wrong, complications can include vascular occlusion (where filler blocks a blood vessel), tissue damage, skin necrosis, infection, scarring, and in rare cases, visual impairment or blindness. These are not scare stories; they are recognised medical complications that every trained practitioner should understand and be prepared to manage.

This is why choosing a reputable, well trained practitioner matters. It’s not just about the before-and after photos. It’s about anatomy knowledge, complication management, appropriate product choice, emergency protocols, and knowing when NOT to treat.

I also think it’s important to remember that even highly experienced surgeons can have complications. Surgery and aesthetic medicine are not exact sciences, and no practitioner can ethically guarantee perfection. What matters is honest assessment, careful planning, informed consent, and putting patient safety above everything else.
