Your pain might not be a just loading or movement problem…

Over the last few years, research has been pointing in the same direction ⬇️

🧠 Tendons and pain can be influenced by your metabolic health, not just training volume.

Here’s what the science is telling us:

▶️ People with insulin resistance or metabolic syndrome have a higher risk of developing tendinopathy even when training exposure is similar.
▶️ Elevated blood sugar (HbA1c), even below diabetic levels, is linked to poorer tendon structure, reduced collagen quality, and slower walking speed.
▶️ Diabetes dramatically increases the risk of tendon pain and rupture, particularly in the Achilles and upper limb tendons.
▶️ High cholesterol and metabolic dysfunction are associated with higher tendon injury rates
▶️ Insulin resistance is linked to lower limb pain, suggesting pain isn’t always purely mechanical
▶️ Metabolic syndrome worsens rehab outcomes, meaning standard loading programs may be less effective if the internal environment isn’t addressed.

Translation:
You can be doing “all the right rehab exercises”…
…but if blood sugar control, lipids, sleep, stress, and recovery are off, your healing in a compromised environment.

This doesn’t mean loading doesn’t matter.
It means loading alone isn’t always enough.

Rehab should consider:
✔️ Progressive mechanical loading
✔️ Metabolic health
✔️ Sleep & recovery
✔️ Stress & systemic inflammation

Train the tissue, but don’t ignore the system it lives in.

Connor had been dealing with hip pain for years.

Previous imaging confirmed arthritis within the joint.
He’d seen doctors and consultants and undergone steroid injections — with no meaningful or lasting relief.

After multiple failed attempts to resolve the issue, he reached out to me.

Following a full assessment, we identified clear mechanical limitations that were loading the hip unnecessarily.
From there, we built a structured plan designed to address those limitations — one that integrated seamlessly into his existing training and progressed in a logical, measurable way.

This wasn’t about chasing symptoms.
It was about maximising function and capacity despite a diagnosis.

Just 3 months in, Connor is now pain free for the first time in years and functioning at a high level again.

Arthritis makes things harder,
But it doesn't make them impossible

If you’ve been told your pain is “just wear and tear” and you’ve been stuck for months or years, this is exactly the kind of case I work with.

DM me “HIP” to find out if a similar approach is appropriate for you.
