"Il trattamento più adatto alla persona che ho di fronte non
è necessariamente il trattamento migliore in assoluto."

E non è nemmeno, quasi mai, il trattamento che preferirei io.
Dopo la diagnosi c'è un secondo passaggio che conta quanto il
primo: capire chi ho davanti e che percorso può affrontare.

C'è chi accetta volentieri un miglioramento parziale, purché
la ripresa sia breve e nessuno se ne accorga. C'è chi ha un
budget preciso, o chi non vuole settimane di recupero. Non
sono capricci: sono vincoli reali, e li considero parametri
della scelta clinica.

Un esempio. Un cedimento iniziale della linea mandibolare si
può affrontare con un lifting, anche deep plane. Ma a volte i
tessuti non si sono allungati: sono scesi perché nelle logge
di grasso della guancia c'è meno volume di prima. Lì la strada
può essere un filler o un lipofilling.

Prima la diagnosi, poi la scelta del percorso insieme alla
persona. Non necessariamente partire a bomba.

"The most appropriate treatment for the person in front of me

And it is almost never the one I would personally prefer.
After the diagnosis comes a second step that matters just as
much: understanding who the patient is and what kind of path
they can realistically take.

Some people are happy with a partial improvement, as long as
recovery is short and nobody notices. Others have a set
budget, or cannot afford weeks of downtime. These are not
whims: they are real constraints, and I treat them as part of
the clinical decision.

An example. Early jowling can be addressed with a facelift,
deep plane included. But sometimes the tissues have not
stretched: they have descended because the cheek fat
compartments have lost volume. In that case filler or fat
grafting may be the answer.

Diagnosis first, then the choice of the path together with
the patient. There is no need to go in all guns blazing.

Lo dico subito in visita, perché è una questione di
indicazione. Una trazione molto marcata si ottiene con un

facelift is for: a different operation, with a different goal.

«Funziona perché mette insieme due cose che ho imparato
separatamente: il lifting deep plane e i fili.»

Per vent'anni ho operato lifting chirurgici lavorando sul piano
profondo, quello dello SMAS. In parallelo inserivo fili di trazione,
prima riassorbibili e poi permanenti. Per anni sono rimaste due
strade separate: la logica della chirurgia profonda da un lato, i
fili dall'altro.

Quello che mancava per farle incontrare era il filo. L'ho trovato in
un filo con un'anima in poliestere, che non si allunga, rivestita di
silicone solido e liscio, e con una struttura tridimensionale a
conetti che ancorano i tessuti senza pungerli.

Con quel filo ho potuto portare la logica del deep plane dove prima
i fili non arrivavano: in profondità, senza aprire una via
chirurgica per arrivarci.

Non è lo strumento a fare la tecnica. È il piano in cui lo si mette.

"It works because it brings together two things I learned
separately: deep plane surgery and threads."

For twenty years I have performed surgical facelifts working on the
deep plane, the SMAS layer. Alongside that, I placed traction
threads — absorbable first, then permanent ones. For a long time
these stayed two separate paths.

What was missing was the thread itself. I found it in a thread built
around a polyester core that does not stretch, coated in solid,
smooth silicone, with a three-dimensional cone structure that
anchors the tissues without piercing them.

That thread let me take the deep plane logic where threads could not
reach before: deep, without opening a surgical access to get there.

The instrument does not make the technique. The plane you place it
in does.

«Il silicone non è un materiale solo: dire "silicone" senza dire quale non significa niente.»

Mi chiedono spesso di cosa sono fatti i fili. Dietro la domanda c'è quasi sempre una diffidenza verso il silicone — e la capisco: da
paziente l'avrei anch'io, se non sapessi come sono fatti.

Il silicone che ha creato problemi è quello liquido, iniettabile:
causava la formazione di granulomi, come tutti gli iniettabili
permanenti, ed è vietato in Italia dagli anni Novanta. L'altro caso
critico è il silicone ruvido, per esempio quello di certe superfici
protesiche: proprio perché ruvido può indurre una reazione
infiammatoria.

Il rivestimento dei fili che utilizzo è invece silicone solido e
liscio: né liquido né ruvido. È un materiale biologicamente inerte,
lo stesso impiegato da oltre sessant'anni nelle protesi d'anca e di
ginocchio.

Quello che conta non è il nome del materiale. È il suo stato fisico
e la sua superficie.

"Silicone is not one single material: saying 'silicone' without
saying which one means nothing."

Patients often ask me what the threads are made of. Behind that
question there is almost always a distrust of silicone — and I
understand it: as a patient I would feel the same, if I didn't know
how the threads are built.

The silicone that caused harm is liquid, injectable silicone: it led
to granulomas, as every permanent injectable did, and it has been
banned in Italy since the 1990s. The other critical case is textured
silicone, such as some implant surfaces: precisely because it is
rough, it can trigger an inflammatory response.

The coating of the threads I use is solid and smooth silicone:
neither liquid nor rough. It is a biologically inert material, the
same one used for over sixty years in hip and knee prostheses.

What matters is not the name of the material. It is its physical
state and its surface.

«Dovrei fare un lifting cervico-facciale a mia figlia di 19 anni?»

È la domanda che mi sono posto mettendo alla prova un gesto che tantissimi pazienti fanno in studio: tirarsi la pelle del viso per mostrarmi "quanto cede". Per anni quel gesto ci ha portato dritti verso la stessa risposta: il lifting cervico-facciale.

L'ho confrontato su tre generazioni della mia famiglia — io (52 anni), mia figlia (19), mia madre (78) — e lo spostamento della pelle è risultato quasi identico nei tre casi. Se fosse davvero eccesso cutaneo, su mia figlia non dovrebbe spostarsi quasi nulla.

La verità è che l'invecchiamento del viso spesso non è (solo) una questione di pelle in eccesso: è una ptosi dei tessuti, cioè un abbassamento della struttura di sostegno, a volte con eccesso cutaneo, a volte senza. Capire quale dei due meccanismi prevale è il primo passo di ogni diagnosi corretta.

"Should I perform a facelift on my 19-year-old daughter?"

That's the question I asked myself after testing a gesture I see all the time in my patients: pinching the skin of the face to show me "how much it's sagging." For years, that gesture has led all of us straight to the same answer — a cervicofacial facelift.

I compared it across three generations of my own family — myself (52), my daughter (19), my mother (78) — and the skin moved by almost the same amount in all three. If it were really excess skin, it shouldn't move much at all on my daughter.

The truth is that facial aging is often not (only) about excess skin: it's tissue ptosis — the supporting structures sliding downward — sometimes with excess skin, sometimes without. Understanding which mechanism is at play is the first step of any correct diagnosis.

«Il denominatore comune resta sempre lo stesso: causare il minor trauma
chirurgico possibile.»

Uso l'HDS Thread Lifting — High Definition SMAS Thread Lifting — da quasi
quattro anni: fili permanenti posizionati sotto lo SMAS, cioè lo strato
profondo che sostiene i tessuti del viso.

È un concetto che nel tempo si è evoluto, ed è diventato HDS Thread Lifting
Plus: ai fili affianco il fat grafting, i dispositivi per il sottocute come
Ignite, oppure un microlifting — un'incisione molto limitata con cui
riposiziono i tessuti verso l'orecchio e rimuovo la cute in eccesso.

Quello che non cambia è il criterio con cui lavoro. Con i fili non ho bisogno
di scollare i tessuti per raggiungere il terzo medio del viso, il solco
naso-genieno, le linee della marionetta, le bande del platisma o la jowl lungo

trauma chirurgico che quella via richiederebbe.

HDS Thread Lifting stands for High Definition SMAS Thread Lifting — permanent
threads placed underneath the SMAS, the deep layer that actually holds the
face up. I have been working with it for almost four years.

The concept has evolved into HDS Thread Lifting Plus, where the threads are
combined with fat grafting, with subcutaneous devices such as Ignite, or with
a microlifting: a very limited incision through which I reposition the tissues

«Vedi che c'è l'angolo?»

È la domanda che faccio alla mia paziente poche ore dopo l'intervento,
ancora in day surgery, mentre si guarda il collo per la prima volta sullo
schermo del telefono.

L'angolo di cui parlo è quello cervico-mentoniero: la linea che separa il
mento dal collo. Quando i tessuti profondi scendono quell'angolo si apre,
il profilo si appiattisce e il collo comincia a raccontare un'età che il
viso non ha ancora.

Nel lifting del collo con fili non asporto pelle. Riposiziono verso l'alto
i tessuti che sono scesi e ricostruisco l'angolo attraverso accessi
minimi, in day surgery: si entra al mattino e si torna a casa lo stesso
giorno.

Una precisazione che faccio sempre, anche quando la reazione è quella che
vedete. Qui si vede il risultato immediato, ancora edematoso: il collo
definitivo si valuta nelle settimane successive, e non tutti i colli si
comportano allo stesso modo. La tecnica si sceglie dopo la diagnosi, mai
prima.

“Can you see the angle now?”

That is what I ask my patient a few hours after surgery, still in day
surgery, as she looks at her neck for the first time on a phone screen.

The angle is the cervicomental angle — the line where the chin meets the
neck. As the deep tissues descend that angle opens up, the profile
flattens, and the neck starts to look older than the face does.

A thread neck lift does not remove skin. I reposition the tissues that
have dropped and rebuild the angle through minimal access points, as a
day-surgery procedure with same-day discharge.

One caveat I always add: what you see here is the immediate result, still
swollen. The final neck is assessed over the following weeks, and no two
necks behave the same way. The technique is chosen after the diagnosis,
never before.

Siamo entrati anche noi medici in un sistema che corre veloce, in cui ci si aspetta che al paziente si venda qualcosa: una visita, un prodotto, un trattamento, un intervento. Ai più giovani viene insegnato che il medico è prima di tutto un imprenditore.

Io questo concetto non lo condivido. Ne faccio parte, come tutti, ma il criterio che seguo resta un altro: prima la diagnosi, poi, solo se serve, il trattamento. A volte la conclusione onesta è che non serve fare nulla, e va detta.

Un intervento non è un prodotto da collocare: è una decisione clinica.

Medicine has been pulled into a fast market where the doctor is expected to sell the patient something: a consultation, a product, a treatment, an operation. Younger colleagues are taught that a doctor is, above all, an entrepreneur.

I don't share that idea. I work inside the same system as everyone else, but my order stays the same: diagnosis first, treatment only when it is warranted. Sometimes the honest conclusion is that nothing needs to be done, and that has to be said out loud.
