HAIRLOSS CASE Part 3
After case diagnosis, treatment plan creation and confirmation, CONTIGENCIES were discussed. Some alopecia treatments are likely to trigger counter-reactions immediately the treatment commences. This MUST be discussed.
There are cases where conviction of healing supersedes any fear of failure or case disappointment whatsoever, and this was such.
“It’s not looking good, but she must heal, our God is able” was a strong undoubted assurance
Most service providers sometimes are unreachable in dire times and the patients’ neglect leads to mistrust and complicates the whole process.
Anyway, True to it, two (2) days later, a whats-app message props. Its something expected from all our patients interms of technical support if need arises. (When we embark on a recovery journey, it is an expectation that the patient need our assistance from time to time till the time they will be able to manage the healing process comfortably at home. There are 'fights' we always encounter long before we proclaim of a successful hair-loss recovery where every person is unique and the reactions are sometimes personalized and never shared).

“Good morning, How long would it take for the PUS to clear off? The WOUNDS are still fresh filled with PUS” end of quote

“Good morning too. That’s what we are starting to “fight” to gain control of your scalp. The condition is not immediately controlled and will take sometime before your scalp gets better. Honestly, it’s a slow process and with time, the attacks will start to get less and less as we progress with treatment and eventually they will clear completely. That’s why you will be coming for monitoring” was a timely assurance
“Its quite uncomfortable. I thought it would be cleared even in a span of two days” she added
“There’s a second oil you are to use (anti-bacterial oil which is used after anti-inflammatory herbal mixture) and they work together and they give you relief. Also note that, and being honest, you have had the problem actively for eight (8) years and it’s not possible to reverse almost a decade’s problem in a few days time. Please note that the condition has created an internal problem (scalp) over time and the tissues can’t just heal in a snap. For treatment to “heal” the scalp we will need proportional timeline. Sorry for the pain you are going through at the moment, and all the discomforts but you will overcome all that soonest” were honest words without any exaggeration
After two (2) more days, a picture pops in my whats-app and immediately noted the excess counter-reaction of her scalp condition to the treatment.
Counter-reactions are expected in most cases and the likelihood is always shared as a contingency. The whole scalp was very itchy and filled with PUS and the scalp was feeling tight
“The scalp is still filled in pus, I cant even sleep well and am starting to get worried” she wrote
To give her assurance and at the same time remain focused on the issue, “The scalp condition is very active making its reaction to the initial treatment high, but in days time it will slow down”
At this point the next appointment for scalp review was quickly agreed to be done seven (7) days after consultation.
In the mid morning, the appointment session was on and the first instinct was that the scalp was getting better, although the scalp was infested with so many small BOIL like, PUS filled vesicles. Big scabs filled the entire scalp. But, something different this time, they were more of being suspended closer to the epidermal layer rather ingrained deeper in the dermal tissue.
“When the scalp gets too much infestation, you can result to using the topical steroid cream and the anti-inflammatory mixture followed by the anti-bacterial cream to arrest the condition, but don’t over do the cream, use it sparingly. Use it only when the scalp is really bad and use it once” was pre-adviced
(She had used the steroid cream and antibiotics combo for years with no much results)
Three days later after the review session, the scalp developed painful and itchy pus filled pimples and the patient resulted in using the steroid cream plus the anti-inflammatory mixture followed by the anti-bacterial cream and the condition lessened.
The scalp was pushed with the two herbal products for the next seven (7) days to the next review session. Its at this time we noted that her all time inflamed LYMPH NODES at the neck area and at her left ear were SHRINKING!! GOOD NEWS!!
BUT…..
She was losing hair rapidly at the hairline just below the two front corners

Something is very wrong here.

HAIRLOSS CASE Part 2
Appointment day comes, a chilly cold morning and the patient shows up dressed in a grey hound, wearing an optimistic smile looking flagile and innocently young.
After a brief preempting, she relaxes and we go through her story to form a basis of her journey so far, a crucial element for a treatment plan creation. Of course, If a medication was used before and failed then, you can't even go there!
After lowering her hood, I noticed the magnitude of the hairline infection. Every single hair literally had an accompanying infection making the hairline appear to have black hairs protrutions attached to white bases, PUS.
"This is gross" I must have said to myself "This is the 8th year of her going through this? In such an age?" A feeling of Sympathy was real
"You will heal, and the pain will go, your hair will eventually grow back. Have faith, God has no impossibility" Looking straight at her with total assurance
The bother is, Some of these scalp conditions and ALOPECIA'S can steal very crucial "hair enjoyment" moments and prevents you from enjoying the God's given gift, "HAIR" like for this young lady's case. In her young adult life, she knows hair as pain.
Getting her scalp back to normal and growing her hair was all she wanted, and the reason she came to us.

Before we started, her then medical treatment cache included:
Oral antibiotics
Topical steroid cream
Medicated shampoo

That's a task

SCALP EXAMINATION
Her scalp had:
Follicular inflammations with infections and pus present from the hairline to the vertex, the temples and the apex
Follicular tufting evident at the vertex
Erythema present especially at the tempral areas
Open sores in the hairline making the scalp very sensitive
High pityriasis (scales throughout the scalp)
Pruritus (very itchy scalp)
Open patches at the temples
Swollen lymph glands, three at the neck and one above the left ear

CASE FINDING
FOLLICULITIS DECALVANS (F.D)

INTERLUDE
Folliculitis decalvans (FD) is chronic, neutrophilic inflammation presentation of cicatrizing alopecia characterized by inflammatory perifollicular papules and pustules.
Today, the etiology of this inflammatory process is not fully understood. However, Staphylococcus aureus bacteria scalp colonization has been implicated as a contributing factor.
The current research is unclear if another primarily sterile process with secondary colonization by Staphylococcus aureus is present, or if Staphylococcus aureus starts the process by causing a strong immune reaction. Another possibility is that Staphylococcus aureus produces toxins that act as superantigens which directly activate the T-cells over the variable domain of T-cell receptors against the hair follicles that domicile the staph bacteria.
POINT TO NOTE, STAPH BACTERIA
PLEASE ALSO NOTE:
Medically, there is no DIRECT KNOWN cure to F.D, but symptoms can be reduced. Treatment varies by person and often includes oral antibiotics and topical treatments that control symptoms and prevent further hair loss. It is not possible to restore lost patches of hair.
No permanent cure has been found for this condition.

TREATMENT PLAN:
1ST, TREAT AND HEAL THE SCALP
2ND REGROW THE HAIR.
1ST.
The patient is on current treatment, which is not effective
DIRECTION;
Stop the medicated shampoo (doesn't help, it only gives temporally relief. We use it as a first aid component)
Immediately stop using oral antibiotics. Orals infiltrates all body's systems risking future resistance.
To phase out topical steroid in about two (2) weeks
PRESCRIPTION
Use anti-inflammatory herbal mixture 1x2
Use broad spectrum oil 1x1
Lightly massage the scalp at comfort level
Use sulphate free shampoo every two to three days for the next two (2) weeks

In 14 days

HAIRLOSS CASE PART 1
We received a case through whatsapp and the first images were immediately shared and this was in early January 2024
(picture quality is as shared by the patient, then)
The story background was an 8 year journey of desparation trying to seek treatment for a very itchy and scaly scalp that developes pus and wounds. Sometimes especially when these is an active attack, the scalp gets painful and sensitive.
On inquiry on previous treatments done, the patient had been to dermatologist and given some control medications but didn't seem to work
Looking at the images shared, several things came to mind.
(This condition or (alopecia) is not an average problem and is likely to be classified as scarring (scar forming) alopecia thats known to be a permanent hairloss condition)
One. The patient is young
Two. there is hairline recession acompanied by patches
Three. there are acute follicular inflammations
Four. presence of massive bacterial infection
Five. the patient had suffered all the years and its likely that she has never enjoyed her hair. Current age: mid 20's

This warranted for a physical examination and diagnosis although i had an idea on what it was but correct diagnosis in hairloss is key
