🌀 BENIGN PAROXYSMAL POSITIONAL VERTIGO (BPPV) One of the most common causes of episodic vertigo is not a problem of the brain — but a problem of where the otoconia are located. In BPPV, otoconia normally attached to the utricular macula become displaced and enter a semicircular canal, most commonly the posterior semicircular canal. When the head changes position: 🔹 The displaced otoconia move within the canal. 🔹 This generates abnormal endolymphatic flow. 🔹 The cupula of the ampulla is deflected. 🔹 Vestibular hair cells are stimulated abnormally. 🔹 The brain receives an inappropriate signal of head movement → brief positional vertigo and characteristic nystagmus. 🎯 Key concept: BPPV is essentially a mechanical disorder of the vestibular system. This is why positional tests such as the Dix–Hallpike maneuver are so important — and why canalith repositioning maneuvers, such as the Epley maneuver, can be highly effective. Understanding the anatomy makes the physiology — and the treatment — much easier to understand. 🧠 Anatomy → Mechanics → Vestibular stimulation → Nystagmus → Vertigo #BPPV #VPPB #Vertigo #VestibularSystem #NeuroOtology Otology ENT Otolaryngology DixHallpike EpleyManeuver VestibularRehabilitation MedicalEducation 12/09/2026
