Can the temporary reduction in testosterone with Prolistem cause side effects?

This is one of the most common questions we receive.

The answer begins with understanding where testosterone works.

Most of the testosterone in the body circulates in the bloodstream. However, inside the te**is, testosterone levels are naturally much higher and play a key role in regulating the environment surrounding the s***m-producing stem cells.

During Phase 1, Prolistem is designed to induce a temporary and controlled reduction in intratesticular testosterone—not complete testosterone suppression. This modest change is intended to help “reset” the signaling environment within the seminiferous tubules, particularly the activity of Sertoli cells, which support s***m development.

Why is this important?

Research has shown that carefully reducing intratesticular testosterone for a limited period can stimulate biological pathways involved in s***matogonial stem cell activation and regeneration. This concept has also been explored in experimental studies investigating the recovery of s***matogenesis.

In our clinical experience, this temporary reduction has been well tolerated, and testosterone levels are restored during Phase 2, when the focus shifts to supporting s***m maturation.

How does Prolistem support men with azoos***mia?

Prolistem is designed to help improve the biological environment inside the te**es—an environment largely regulated by Sertoli cells.

Sertoli cells are often called the “nurse cells” of s***matogenesis because they protect, nourish, and support germ cells throughout their development into mature s***m.

In our research, we observed that s***matogonial stem cells obtained from men with azoos***mia were still able to survive and grow when placed in a more supportive environment outside the te**es. This suggests that, in some patients, the stem cells may retain developmental potential, while the surrounding testicular environment—particularly Sertoli-cell support—may be impaired.

This finding supports Prolistem’s strategy: rather than focusing only on the germ cells, the program aims to support and improve the Sertoli-cell-controlled environment required for s***m development.

Prolistem targets the environment that supports s***matogenesis—helping create better conditions for s***m production before microTESE.

Results may vary between patients. Prolistem is intended for selected men with non-obstructive azoos***mia under medical supervision.

New Clinical Data Presented at ESHRE 2026

We are pleased to share new clinical data presented at the ESHRE 2026 Annual Meeting evaluating the effect of Prolistem in men with non-obstructive azoos***mia (NOA) who had previously undergone two unsuccessful microTESE procedures.

In this retrospective clinical study conducted at Momart IVF Center (Turkey):

29 men with NOA and a history of repeated failed microTESE received Prolistem treatment.

After treatment, 3 of 29 patients (10.3%) had s***m detected in their ej*****te.

Among the remaining patients, 13 underwent a repeat (third) microTESE, and 3 patients (23.1%) achieved successful s***m retrieval.

Overall, s***m became available for assisted reproduction in 6 patients, with three ongoing pregnancies reported during follow-up and no treatment-related adverse events observed.

These findings are encouraging because they suggest that a subset of men who had previously failed two microTESE procedures may still achieve s***m retrieval following Prolistem treatment. While the study is retrospective and relatively small, its results are consistent with previous clinical experience evaluating Prolistem in carefully selected men with non-obstructive azoos***mia.

We thank the investigators at Momart IVF Center for their valuable contribution to advancing research in male infertility and look forward to additional prospective studies that will further define which patients are most likely to benefit from this approach.
