When this patient came in, we noticed a few key things happening beneath the surface: highly inflamed skin leading to collagen breakdown leading to volume loss and a few structural changes. Stressed skin can compromise overall tone and texture. So rather than approaching treatment with a predetermined look in mind, we started with a question: what does this face already do beautifully, and how do we support that?
Here’s what intentional filler placement looked like for this patient:
Temples — Restoring lost volume here lifts the entire upper face, softening the appearance of hollowing that often gets mistaken for tiredness.
Midface — Addressing collagen breakdown in this area rebuilds the scaffolding that keeps features looking supported and defined.
Nose — Subtle refinement to improve symmetry and proportion, without altering the natural character of the face.
Lips — Dissolving and improving with light enhancement to restore balance.
We also worked to calm the underlying inflammation, because healthy skin is the foundation everything else is built on. Dedication looks good on her! Remember, it’s a marathon not a race…. And the goal is never to make someone look like a different version of themselves.
Let’s talk about thread lifts and the horror stories you’ve seen online? Most trace back to outdated techniques, poorly selected patients, and first generation nonabsorbable materials like polypropylene. Those early threads carried real risks: long-term pain, protrusion, and removal that required surgical incisions. That criticism was fair … for its time.
But thread lifting didn’t stop there. The field has evolved through four generations of technology, moving from manually cut cogs to precision-molded modern designs, and from permanent materials to absorbable ones — P.DO, PLLA, and PCL. Today’s P.DO threads dissolve gradually over 6+ months, all while actively stimulating collagen and elastin production, triggering a wound healing response that leaves the tissue firmer even after the thread is fully absorbed.
Here’s what devices and fillers simply aren’t designed to do: mechanically reposition sagging tissue. Thread lifting works by establishing a fixing point in stable fascial tissue and physically anchoring loose, descended soft tissue back into a lifted position, with true directionality. When done right, it engages the right layer. It creates a reinforcement effect. It generates myofibroblast activity that actively contracts and tightens tissue over time.
That is a different mechanism entirely.
Do threads require precise patient selection? Yes. Proper anatomical knowledge? Absolutely. The right thread design for the right indication? Without question.
But when those boxes are checked, beautiful results. I still believe in this procedure. I stand behind it.

Those shadowy dips beside the chin are called the pre-jowl sulcus. Everyone’s terrified that non-invasive technology (that's been around for awhile) either “does nothing” or “melts your natural facial fat.” But the trick is using the right frequencies & protocols to target skin depth without compromising volume.

HELPFUL TIPS YOU DONT OFTEN THINK ABOUT
•The “Saturate & Snip” Trick: Hanging skin catches on pillowcases, face masks, and shirts, causing accidental tearing. You can heavily coat the hanging skin in a thick layer of a healing ointment for 5 minutes to completely soften the edges. Once the dead skin is fully saturated and pliable, you can safely use sanitized eyebrow scissors to snip the loose part without pulling the anchored root.
•Beware of “Micro-Friction”: Most people think “picking” only means using their fingers. In reality, side-sleeping on a standard cotton pillowcase creates enough micro-friction to prematurely rub off peeling skin during the night. Be mindful your first few days post peel and maybe switch to a high-slip silk or satin pillowcase for the first 7 days to let the skin shed on its own timeline. Not always necessary.
•The Yawning & Chewing Factor: Aggressive facial expressions, wide yawning, or eating highly crunchy foods can prematurely crack the dry, tight “mask” of dead skin before it is biologically ready to separate. For the first 3 to 4 days, be mindful and minimize exaggerated facial expressions or opt for softer foods to prevent premature tearing around the mouth and jawline.

I still meet women in their forties and sixties who are told their symptoms are “just stress” all the time. There is way more to that when it comes to the estrobolome.
Your liver processes estrogen in two phases: breaking it down, then tagging it for disposal. before sending it to the gut to exit. But if your gut microbiome is out of balance, certain bacteria produce an enzyme called beta-glucuronidase. This enzyme acts like biological scissors, cutting the tag off and returning that active estrogen straight back into your circulation.
One enzyme is the difference between estrogen leaving your body and estrogen taking another lap. This is why two women with identical estradiol levels can live in completely different bodies. The standard lab measures the pool. It doesn’t measure the drain. And it certainly doesn’t tell you which metabolic pathway your estrogen took on the way down, the protective pathway or the proliferative one.
Those are three entirely separate clinical questions, and a standard blood panel only answers one.
