What if the pain your patient is experiencing isn’t coming from just one mechanism?
Pain can be complex. Burning, aching, inflammation, sensitivity or nerve-related symptoms may not always tell the same story.
That’s why pain management shouldn’t begin with simply adding more ingredients to a formulation. It should begin with understanding the patient.
When clinically appropriate, a provider may consider a compounded topical formulation designed around an individual patient’s needs. But more ingredients do not automatically mean better results, and the evidence varies depending on the ingredients and formulation being used.
For us, thoughtful compounding means asking the right questions first: What type of pain are we addressing? What has the patient already tried? And is there a genuine need that available medication options aren’t meeting?

Our Founder and President, Dr. Ndidiamaka “Didi” Okpareke, recently returned from Compounders on Capitol Hill 2026, hosted by the Alliance for Pharmacy Compounding (APC).
As a pharmacist, healthcare entrepreneur, and candidate for the U.S. House in New Mexico’s 1st Congressional District, the experience reinforced something Dr. Didi sees every day in healthcare:
Access to medication isn’t partisan.
Patient safety isn’t partisan.
A pharmacist being able to care for a patient isn’t partisan.
Among the major conversations in Washington was desiccated thyroid extract (DTE). APC reports that approximately 1.5 million Americans rely on animal-derived thyroid medications, making the regulatory pathway for continued patient access an important issue.
Peptides were another major discussion, including how appropriate access can exist within a regulated healthcare system while prioritizing science, patient safety, clinical oversight and ingredient quality. The broader peptide conversation has also received attention from HHS Secretary Robert F. Kennedy Jr.
Dr. Didi also visited the Washington office of U.S. Senator Ben Ray Luján, where she spoke with his health policy staff about compounding pharmacy and healthcare issues affecting New Mexico and the country.
The biggest takeaway?
Healthcare works best when we are willing to work across the aisle.
Showing up, listening, educating and finding common ground is a good place to start.

Our Founder and President, Dr. Ndidiamaka “Didi” Okpareke, had the opportunity to meet with Peter Obi for a meaningful conversation about healthcare, leadership, and the future of Nigeria.
A major part of their discussion centered on what it would actually take to build a functional and sustainable healthcare system—from the cost of building and properly equipping facilities to expanding access to specialty services such as dialysis.
For Dr. Didi, a first-generation Nigerian American and healthcare leader, the conversation was particularly meaningful. It was an opportunity to bring her experience in pharmacy and healthcare into a broader discussion about building systems that can truly serve people.
But healthcare was only part of the conversation.
They also discussed leadership, accountability, corruption, strong institutions, and the importance of free, fair, credible, and safe elections.
Peter Obi, a businessman and former Governor of Anambra State who was the Labour Party’s presidential candidate in 2023, has remained active in Nigeria’s national conversation around governance, economic development, healthcare, education, and institutional reform.
Nigeria has no shortage of talented people, professionals, or ideas. The bigger challenge is creating systems—and leadership—that allow that potential to translate into real progress.
For Dr. Didi, it was a meaningful opportunity to contribute to that conversation as a pharmacist, healthcare entrepreneur, and Nigerian American who remains deeply connected to Nigeria’s future. 🇳🇬
Better healthcare requires better systems. And better systems require leadership, accountability, and the willingness to do what is right.

Our very own Dr. Ndidiamaka “Didi” Okpareke will be heading to Washington, D.C. for Compounders on Capitol Hill (CCH), September 14–16, 2026.
Hosted by the APC Alliance for Pharmacy Compounding (APC), CCH brings compounding pharmacy professionals together to advocate for the profession, engage with policymakers, and help ensure that the voices of pharmacists and the patients they serve are represented in conversations shaping the future of compounding. The event culminates in congressional visits on Capitol Hill.
For Dr. Didi, this is about more than representing Olive Tree Compounding Pharmacy. It is an opportunity to stand alongside fellow compounders and advocate for responsible pharmacy compounding, sensible regulation, and continued patient access to personalized medications when they are clinically needed.
The decisions made in Washington can have a very real impact on what happens inside our pharmacies, in our providers’ practices, and ultimately in our patients’ lives.

When we think about medication, most of our focus naturally goes to the active ingredient. 💊
But whether it’s a capsule, cream, tablet, or liquid, the finished product contains much more than just the primary drug.
Enter excipients—the inactive ingredients responsible for stability, appearance, texture, taste, or delivery. Depending on the formulation, these can include:
✨ Dyes & fillers
✨ Binders & preservatives
✨ Flavoring, sweeteners, or oils
For most people, these ingredients work behind the scenes without any issue. But no two patients are exactly alike.
A patient might tolerate an active medication perfectly fine, yet experience a sensitivity or allergic reaction to an inactive component. The FDA even recognizes that custom compounding is often necessary when a commercially available product contains a dye or filler a patient cannot tolerate!
When an adverse reaction happens, we shouldn’t automatically blame the active drug or ignore the inactive ingredients. The entire formulation deserves a look:

🔹 Which excipients are present?
🔹 Has the patient reacted to similar formulations before?
🔹 Is there a documented allergy or sensitivity?
🔹 Is an FDA-approved alternative available?
Asking these questions helps providers and pharmacists uncover what’s really going on.
While compounding is not a replacement for FDA-approved options and must always be grounded in clinical need, it provides a tailored solution when standard options fall short—like removing a problematic allergen.

Let’s talk about bioidentical hormones, because the word “bioidentical” can be confusing.
Bioidentical doesn’t automatically mean “natural,” safer, or better. It means the hormone is chemically identical to a hormone produced by the human body. In fact, FDA-approved bioidentical hormone medications already exist.
So where can a 503A compounding pharmacy fit in?
It comes down to the individual patient.
Sometimes a provider determines hormone therapy is appropriate, but an available medication may not meet a patient’s specific needs. Maybe there’s an ingredient they cannot tolerate, or a commercially available strength or dosage form isn’t appropriate for their situation.
When clinically appropriate and permitted, a compounding pharmacist can work with the prescriber to evaluate a patient-specific formulation.
That’s what personalization means to us.
Not that customized is automatically better.
Not that “natural” automatically means safer.

What if the problem isn’t the medication, but swallowing it? 💊
For some patients, taking a tablet or capsule can be a real barrier to treatment. Children, older adults, and people with swallowing difficulties may struggle with conventional dosage forms.
And simply crushing a tablet isn’t always the answer. Some medications should not be crushed, split, or altered because doing so can change how the medication is released or works.
That’s where personalized pharmacy care can make a difference.
When clinically appropriate, a prescriber and compounding pharmacist can evaluate whether a patient-specific dosage form may better meet an individual need when an appropriate commercially available option isn’t suitable.

Creatine is often associated with bodybuilders and elite athletes, but the science behind it is much broader than gym culture.
Stored primarily in skeletal muscle, creatine helps replenish phosphocreatine, which supports the rapid production of ATP, the energy muscles rely on during short, intense efforts such as resistance training, sprinting, and repeated high-output exercise.
It is also one of the most extensively studied sports nutrition supplements available.

Over the past several years, peptide therapies have generated significant interest from patients, providers, researchers, and regulators alike.
The conversations have been passionate.
Sometimes controversial.
But perhaps we’ve been asking the wrong question.
Instead of asking,
“Should peptides be available?”
Perhaps we should be asking,
“How do we responsibly evaluate emerging therapies while protecting patients and preserving access to individualized care?”
That is why this week’s activity at the Pharmacy Compounding Advisory Committee (PCAC) is worth paying attention to.
The committee’s recommendation to include BPC-157, KPV, TB-500, and MOTS-c on the proposed Section 503A Bulks List does not make these substances FDA-approved therapies.
Nor does it immediately authorize their use in compounded medications.
What it does represent is something equally important.
A willingness to continue evaluating whether certain therapies deserve a place within a regulated, patient-specific healthcare framework.
That distinction matters.
Personalized medicine has never been about offering every new therapy.
It has always been about balancing three responsibilities:
• Scientific evidence
• Regulatory oversight
• Individual patient need
Those principles should never compete with one another.
They should work together.
As compounding pharmacists, we believe innovation should move forward—but it should move forward responsibly, transparently, and with patients at the center of every decision.
Whether the FDA ultimately accepts or rejects these recommendations, one thing is becoming increasingly clear.
Healthcare is moving away from a one-size-fits-all model.
Providers are asking more individualized questions.
Patients are seeking more personalized solutions.
And the regulatory conversation is evolving alongside them.

For too long, healthcare has relied on a simple assumption: if a medication works for most people, it should work the same way for everyone.
