MELASMA 101 — IT’S NOT JUST ANOTHER DARK SPOT
There is SO much misunderstanding about melasma, so let’s talk about what is actually happening beneath the surface. 👇🏻
FIRST — WHY IS MELASMA DIFFERENT FROM “DARK SPOTS”?
A dark spot such as post-inflammatory hyperpigmentation (PIH) usually develops because the skin has experienced an inflammatory event, injury, or cumulative UV exposure — acne, a wound, irritation, a burn, a procedure, picking at a pimple, or repeated sun exposure, for example.
But here is something important to understand:
It is the melanocyte’s job to help protect the skin.
When the skin is exposed to UV radiation, melanocytes produce more melanin. That melanin helps absorb and disperse UV radiation and is part of the skin’s natural defense mechanism.
This is why we tan.
A tan is essentially the skin saying, “I need help protecting myself.”
I often describe it as the skin’s cry for help.
So when inflammation, injury or UV exposure occurs, the melanocyte responds. It is doing what it is designed to do — protect the skin by increasing melanin production. As we age, the 'even' tan we use to get looks more spotted, brown spots.
But when that response leaves excess pigment behind, we can end up with PIH.
PIH is generally a response to something that happened to the skin.
And once we identify and correct the underlying trigger, calm the inflammation and appropriately treat the pigmentation, PIH can often be corrected without necessarily becoming a lifelong recurring condition.
MELASMA IS DIFFERENT.
Melasma is a chronic pigmentary disorder in which the melanocytes are more active and produce excess melanin.
It is influenced by multiple factors, including UV exposure, visible light, heat, hormones and hormonal influences, genetics, and other signaling pathways.
And this is one of the biggest differences between PIH and melasma:
Hormonal influences are a major factor in melasma. They are not considered a primary driver of PIH.
That difference matters when we treat them.
SO, HOW DOES MELASMA ACT DIFFERENTLY THAN PIH?
Think about it this way:
PIH:
Something happened → inflammation, injury or UV exposure occurred → melanogenesis was stimulated → pigment was produced.
MELASMA:
The melanocytes are more reactive/active → multiple triggers continue to stimulate them → they continue producing excess pigment, because of pregnancy, breast feeding and hormonal influences, and menopause.
And this is why melasma is an ongoing journey.
Once melasma is present, the tendency toward excess pigmentation can continue, even after we have successfully lightened what we see on the surface.
That means we aren't just correcting pigment.
We have to continually manage the factors that over stimulate the melanocytes.
That is why I don't approach melasma as simply, “Let's lighten the brown.”
WHAT IS OUR GOAL WHEN TREATING MELASMA?
Our goal is to continuously help stabilize the melanocyte and reduce its tendency to overproduce melanin.
We need to work toward controlling the melanocyte activity and the process responsible for producing excess melanin.
And this is an ongoing process — because with melasma, we aren't just correcting an effect that happened once.
We are managing a tendency for the melanocytes to continue overproducing pigment.
That is a completely different mindset from simply chasing the pigment that you can see on the surface.
WHY IS A TYROSINASE INHIBITOR SO IMPORTANT?
Tyrosinase is a key enzyme involved in the production of melanin.
So when we use a tyrosinase inhibitor, we're targeting an important step in the pathway that creates pigment.
I like to think of it almost like putting those overactive melanocytes in a short time-out — helping slow down the pigment-making process WHILE we work on correcting the pigmentation that is already there, so we aren't continually stimulating more pigment production.
We're not simply trying to erase what we see.
We're trying to manage the process that is creating what we see.
AND THIS IS WHERE MELASMA TREATMENT CAN GO WRONG.
Heat matters.
Heat can be a trigger for melasma. Think saunas, excessive heat and humidity during hot summer months, hot environments, and even hot flashes.
Inflammation matters.
For acne-prone skin, inflammation can be driven by excess sebum (oil), clogged follicles and an overactive sebaceous environment.
So getting acne under control isn't just about treating the pimple you see. We need to address what is driving the acne and inflammation in the first place.
Trauma matters.
And yes — picking a pimple is trauma to the skin.
So is aggressively squeezing, scratching, over-exfoliating, or unnecessarily irritating the skin.
This is why procedures such as microneedling, chemical peels, IPL and lasers require careful consideration when treating someone with melasma.
A procedure isn't automatically wrong for someone with melasma — but the skin needs to be properly assessed and prepared, and the treatment needs to be carefully selected.
SO REMEMBER:
PIH = identify and control the cause of the inflammation or injury + correct the pigment.
For example:
👉🏻 Acne → control the acne and inflammation at the source + correct the resulting PIH.
👉🏻 Picking → stop the trauma + allow the skin to heal + correct the resulting pigment.
👉🏻 Excessive UV exposure → protect the skin from further UV exposure + correct the resulting pigmentation.
MELASMA = manage the triggers + stabilize melanocyte activity + inhibit melanin production + correct existing pigmentation + protect against UV and visible light.
And with melasma, heat, hormones, light, inflammation and trauma all matter.
The goal isn't simply to make the pigment lighter.
The goal is to get the melanocyte under control so it isn't constantly being encouraged to make more.
TREAT THE CAUSE — NOT JUST THE EFFECT. 🤍
